Baby Girl---- Unit #896127
Date of Birth Nov. 24/73 at 1510----- Date of Death Nov. 25/73 at 1510 hours
Birth Weight 1030 Grams.
Maternal history as attached.
Premature infant born at 1510 hours. Apgar Score at 5 minutes was 4 and at 10 minutes was 8. Resuscitation - 02 by mask and pressure.On examination - birth weight -
- 1030 grams.
- head circumference - 26 cm.
- C.H. length - 37 cm.
The general physical examination was negative. The infant was brought down from the case room and in an isolette with the oxygen running at 80%. An astrup taken at 45 min/age:
pH - 7.23
pCO2 - 47 mm. Hg
B.E. - 8 mEq/1
p02 - 61mm Hg.
The infant lived for 24 hours and in that of increasing respiratory failure. At 22 hours of age the baby was intubated and put on the Bird Respirator with 18 cm/H20 and a rate of 60/min. At this time the baby was noticed to be hypothermic, so a heat lamp was put over the isolette. Repeat chest x-ray confirmed the diagnosis of Hyaline Membrane Disease. An astrup taken at 22 1/2 - hours of age in 100% ` 02 showed:
pH - 6.65
pCO2 - 133 mm/Hg
B.E. - -30 mEq/1
p02 - 11 mm/Hg
The infant deteriorated and died at 24 hours of age.

Delivered November 24/73 at 1510 hours ---- Neonatal Death - female
Present history: This 43 year old, obese mother of three presented on Nov. 16, 19'73, distraught by the fact that she was pregnant and requesting termination and sterilization, She spoke very poor English. Last Menstrual Period - May? Menses irregular. No contraception practised recently.
Past history: Difficult to obtain. Dec. 1971 for Menorrhagia. Three children age 14 to 21 years old. All S.V.D.F.T. No complications.Examination: in office revealed a very obese lady with a fundus below the umbilicus and a gestation 18 weeks? She was admitted for termination by intra-amniotic saline on November 23rd, 1973 when 10 cc's liquor were removed and 200 cc's 20% saline introduced.
November 24th, 1273 Patient delivered a live female infant, weight 1,030 G. at 1510 hours on the ward. The baby was sent to C.R. The patient was noted to have a large oedematous umbilical cord and placenta which was retained and removed under General Anaesthetic an hour later. The uterus was curetted empty using a large uterine curette. Patient sent to P.A.R. in satisfactory condition.
